That frequency points to a systemic problem, not a personal failing. Here's what that teaches us about design, mistake-proofing, and continuous improvement.

That might sound like a freak event. But according to Airbus data, inadvertent slide deployments (ISDs) happen about three times a day worldwide.
Three times a day. Or 40 times a year? Across the global fleet.
Either way, that's not rare. That's a systemic problem.
When “human error” shows up this often, it's evidence of a system design problem, not an individual failure.
The most common cause? A door is opened while it's still “armed” — meaning the evacuation slide is ready to deploy if needed in an emergency. In this case, a 26-year veteran lifted the handle after arming the door, and the system did exactly what it was designed to do: it deployed the slide.
So yes — it was “human error.” But it was predictable human error, the kind that strong systems are designed to anticipate and prevent.
Designing for Predictable Human Mistakes
Even with procedures, checklists, and cross-checks, people get distracted, fatigued, or rushed.
Mistake-proofing — or poka-yoke in Japanese, designing processes so errors are hard to make — is never perfect when it depends solely on humans to execute every step correctly.
This is where design matters. Some newer Airbus aircraft now incorporate a feature called “Watchdog,” developed by Airbus subsidiary KID-Systeme. The system uses a proximity sensor at the door handle that flashes a light and sounds an alert if someone reaches for it while the door is still armed. It's a clever, layered defense — a system-level cue that interrupts a predictable human mistake before it becomes a $70,000 event.
That's not about blaming the crew. It's about designing for humanity.
What Lean Thinking Teaches Us About “Human Error”
In Lean thinking, we don't ask, “Who messed up?” We ask, “How did the system make this mistake possible?”
If an error occurs a few times a decade, it might be an anomaly. If it happens three times a day, that's a signal the system needs to change.
Good systems don't rely on perfect people. They assume mistakes will happen — and they're built so those mistakes don't lead to expensive, dangerous, or embarrassing outcomes.
Do the airplanes have clear visual indicators that can't be missed? There are times, of course, when an “armed” door needs to be opened in an emergency. So you can't prevent the opening of an armed door. But how do you better mistake-proof the opening of a door that a flight attendant erroneously thinks is unarmed when it's still armed?
Aviation has long been a model for safety and continuous improvement. Yet even here, human factors still remind us: the work of improvement never ends.
This isn't just an aviation story — it's a leadership lesson about how organizations respond to repeated errors.
This same pattern appears in healthcare, manufacturing, and software: repeated mistakes labeled as “human error” are usually design flaws hiding in plain sight.

- Do our safeguards and cues make errors less likely — or more likely to go unnoticed?

Continuous improvement begins with humility — the recognition that our processes can always be made safer, simpler, and more error-resistant.
If something happens three times a day, the question isn't who made the mistake — it's how the system was designed to allow it. And why it hasn't been fixed yet.
Leader heuristic: If an error happens often, design failed. If it happens rarely, investigate. If it keeps happening, redesign.
