Podcast Follow Up: Kaizen Forms

I received a follow up question after my first Podcast with Norman Bodek, about where to find a template or example of a kaizen form.

One suggestion is to take a look at Norm's book The Idea Generator. Does anyone have an example of a kaizen form to share? If so, email me at leanpodcast@gmail.com and I will post the file for others to download.
