Good letters to the editor on this topic in yesterday's WSJ:

Donald Berwick of the IHI wrote, in part:

"...mindless enforcement of standards is dangerous..." but he also pointed out a study that showed only 56% of patients got recommended care.

Lawrence McAndrews wrote:

"our health-care system's quality, costs, and individual patient outcomes suffer far more from unnecessary variability than from over-standardization."

Peter Gentling wrote:

"... these rigid rules cannot fit the complexity of the human condition..."

Joel M. Zinberg (M.D. and J.D.) wrote:

"'pay-for-performance' is really 'pay-for-process."

and Daniel Keays complained that the mandates don't take long-term effects into account, that his hospital caves to the short-term pressure to give certain anti-biotics, but that might be bad for the long-term (causing antibiotic resistance).

I tempt fate every time I mention Marriott while staying at a Marriott hotel (as I had been Wednesday night).

With their vaunted standardized process, there was a minor defect in their room cleaning process... I ended up with no shampoo bottle in my room for Thursday morning...

Where can I find one of those robotic standardized factories?
I'd love to work there, but I suppose once you set-up one of 200 different part numbers on an NC lathe, you've set-up them all, right?

Mark;
People confuse mindless conformity with "set a standard, follow a standard, look for a better way". First reaction is to make rules and incentives. Check out Barry Schwartz's TED talk which I posted. Maybe you have seen it before. http://freshperspectives.typepad.com/fresh_perspectives/2009/02/great-ted-talk.html
Mike
