It seems like the hospital could mimic the new bottle color scheme on their old stock but putting a colored sticker on the lid.

I'm approaching this from a tech perspective - the implementation of barcoding, except on the most basic of levels, would likely be immense (integration with existing systems, development of new systems, etc.). I'd think cost wise the place to scan would be the pharmacy in the hospital (stationary, single point).

Basically, I'm imagining, a scan on departure that tied the medicine and dose into the patient's file.
Outside of initial time/effort/cost of implementation I see two potential issues -

1. Doesn't eliminate the medicine being mixed up after it has left the pharmacy (and having a barcode scanner for every nurse or patient to be used at application would be absurd).

2. Bacodes aren't 100% - as we all know from purchasing groceries.

Still, seems like a good idea that deserves more investigation/testing than it's presently getting.

I'm sure someone out there could start a nice little start-up on that idea.

Barcoding *can* be an effective strategy, but there are also (unfortunately) clever workarounds that people can employ to save time. Not sure why bottles like this aren't barcoded.

Why arn't barcodes used? It would be very easy to interlock to the patients medical records and flag possible errors (based on age, sex, other medications, etc).
